Why are we changing to the Total Patient Triage Model? We want to ensure that our appointments and resources are being maximised and used appropriately to create extra capacity and reduce waiting time.

This is in line with the NHS Modern General Practice Model.

All requests (except nurse/HCA appointments) will be via our Total Patient Triage System.  

Instead of calling the practice, patients will be asked to complete an online form via our website.

Total triage process

Our phone lines will still be open but will be for those patients that are unable to submit an online form or if they are booking an appointment with our nurse/HCA.

The form only takes a few minutes to complete and submit.

You will be able to submit an admin request 24/7.

You will be able to submit a medical request from 8am Monday – Friday.

Do I need to register for Patient Triage?

No, there is no need to register to use Total Patient Triage. If you are a patient at Fairbrook Medical Centre, you complete the form via our website and once submitted it is sent to the practice. These details will be matched against our clinical system.

What happens after I submit the online consultation form?

Depending on the information provided and the nature of your request, you may receive further communication via text message or phone call. This could involve requesting additional information, providing advice or guidance, or scheduling an appointment with a clinician.

Can I request a specific clinician?

Yes, you can specify if you have a preferred clinician when submitting your online consultation form. Whilst we endeavour to match you with your preferred clinician, we cannot guarantee it due to availability and scheduling constraints.
